What is a Dental Implant?

A dental implant is a prosthetic replacement for a missing tooth. As we know natural tooth structure consists of a root and a tooth, so in an implant procedure, we replace the root with an implant and replace the tooth with a crown or denture. It is that staright forward ! The implant itself provides a strong foundation for the crown and this can be either fixed or removable. Dental implant procedures are carried out in the dental office and are done in 1 or 2 stages.

So what exactly is a Dental Implant?

Dental implants are predominantly made of titanium. Titanium is a metal that is bio-compatible -(compatable with a living tissue or a living system that is not toxic, injurious, or physiologically reactive and is rejected by the immune system.) All in all it means that titanium offers the best strength and durability. Dental implants are made in different sizes to suit the gap to be filled and for compatability to the bone.

Dental implants are the most popular option for teeth replacement as they are like new teeth and are strong and stable – they look and feel like your own natural teeth. They give back the confidence and self esteem to smile again.

There are three parts of an implant:

1– the implant device itself (which is placed directly into the bone)

2– the abutment piece that connects the implant device to the third part

3– the overlying crown or denture.

Are dental implants for me?

Dental implants are the best option for people who have missing teeth. They offer the best permant solution for tooth replacement and is most beneficial in regard to oral care and hygiene.

Dental implants are and option for people who have:

  • A single tooth is missing – instead of a bridge,
  • Several teeth are missing – instead of bridges or partial dentures
  • All teeth are missing – instead of dentures
  • If a person is unhappy with their smile and want to renew their appearance

How do I know if I am a suitable candidate for dental implant?

Almost everyone is a candidiate for dental implants unless there are areas of untreated disease of the teeth , gums or bone. More than likely a dentist will not carry out the treatment until these problems have been treated as they can affect the success of the implant surgery. So if you are considering having dental implants, and think that you may have dental problem, contact your dentist to schedule an oral exam it only takes a about 30 mins and x-rays should be included. If you are considering to have dental work abroad to save some money, it is a good a idea to have a quick exam and x-ray at home before travelling. This will then rule out any problems that may prevent you from having the treatment abroad.

Dental Implant Scenarios

Some of the reasons loss of teeth ocur can be the cause of several reasons. These include losing teeth by an illness or accident, severe gum disease or root inflammation and tooth decay are also causes tooth loss .

In some cases, where teeth are missing from birth may be that teeth are not developed due to genetic reasons . Therefore , gaps are created between the teeth , making it uncomfortable talking and eating . Implants are the best permanent long-term option for teeth replacement. It provides a better solution compared to bridge and dentures.

In the absence of one tooth

When a natural root is lost and leaves a gap then an implant is the ideal solution. Weather there is one or more inplants to be done , the procedure for each implant is exactly the same Unlike a dental bridge, the area where the implant is to be place is the only area that will be affected. The teeth on either side of the gap will not be touched. Whereas with a bridge, the teeth on either side of the gap need to be cut down for the bridge to fit. After the implant has been placed and has fused to the bone over the healing period the crown is then fitted.

In the absence of multiple teeth

An implant bridge can be used when there are multiple teeth missing together. Implants are placed the same way a single implants and the implant bridge is placed. An implant bridge replaces the support lost as a result of missing teeth, is placed from implant to implant and avoids the need to drill the adjacent teeth as the bridge requires the same strenght from implant to implant. Implants are placed in strategic positions to replace the missing teeth. When the implants are stable and ready for loading, abutments can be attached to the implants that will connect the final bridge to the implants. An impression is made recording the contours of the abutments or the position of the implant tops. The implant bridge is then made and placed using dental cement or screws.

If there are multiple teeth missing in different areas of the mouth, then single implants can be placed with crowns on top. Again, unlike a bridge, the teeth on either side of the gap are not touched.

In the absence of all teeth (upper and lower jaw)

Option 1

Option 2

In the first situation (option 1) where all teeth are missing, a full set of implants can be placed. This would require a Max of 8 implants on the upper jaw and Min. of 6 implants on the lower jaw. Two sets of 14 crowns are then placed. 14 crowns are placed on the top jaw and the lower jaw.

The second option is to have implants and a click denture. Four implants are usually placed on the lower jaw and four implants are placed on the upper jaw. Then denture is then placed using a click system . The denture is clicked on the implants , setting your dentures firmly in place. The denture( click system ) can be removed and thus easy to maintain . The implants stop the shrinkage of the jaw completely . If the bone and gums are healthy , one can choose several implants and crowns choose . dental Implants have more advantages in relation to other teeth replacement options. For instance implants supporting bridges and dentures have more confidence and the to keep from biting and chewing has more security capacity.Depending on the number of implants placed , the connecting device that your new teeth will be tightened to the implant , or it can support a round ball anchor to which a prosthetic ratchet system is adjusted .

In some cases , where teeth have been missing for a long period of time, the jawbone may shrink . Shirnkage of the bone on the upper jaw may cause sinus to drop . Therefore, a bone graft or sinus lift will need to be carried out before an implant procedure. This can be clarified with an x-ray. This is one the reasons it is a good idea to have an x-ray taken before you travel for a dental implant treatment. Although it is never a problem when you arrive at your destination and need this done, it just means you can plan better and add in the cost for this simple surgery. There is a healing a period after treatment before you can have the implants placed.

Adjunctive procedures

Bone Graft/ Bone Augmentation

Most often the jawbone will shrink where areas with teeth that have been missing for a long time, and/or have been prosthetically replaced with removable dentures. There is not enough room for the implant to be placed where the bone has reabsorbed.

There needs to be sufficient jawbone for a patient to have a dental implant. If there is not enough bone or if the tissue is too soft for the implant to be placed a bone graft is neccessary. A bone graft is a replacement of a missing bone. The dentist inserts synthetic bone into the area where the implant will be placed and leav it to heal for about 6 – 8 weeks. When all is good , the implants can be placed.

Sinus Lifting

Sinus lifting is required when there is insufficient bone available to securely place the dental implant. When upper back teeth are missing for a long period of time bone in that area naturally shrinks or resorbs. Your sinuses expand to occupy the empty space where the bone used to be. The sinuses must be lifted in order to create space for placing additional bone in that area. The bone is added between your jaw and maxillary sinuses.

Implants may be contra-indicated if a patient is taking intravenous and oral bisphosphonates as this may put a patient at a higher risk of osteonecrosis. Osteonecrosis is a delayed healing syndrome .

Uncontrolled Type II diabetes is a significant relative contraindication as healing following any type of surgical procedure is delayed due to poor peripheral blood circulation

What are the benefits to having dental implants?

There are many benefits and advantages to having dental implants placed.

- Improved oral function
- Prevent/reduce bone loss
- Able to eat what you want
- Better dental hygiene
- Improved facial appearance
- Boost your confidence and self esteem
- No worries about slippage or problems with eating
- Convenient
- Long lasting
- Comfort

The disadvantages to having implants are:

- Time
- Expense

Before the procedure is carried out:

Before having the procedure, a full oral examination is carried by your dentist. Whereby the gums, teeth and bone is examined paying extra attention to the area being treated. An x-ray is taken by either and OPG (panoramic x-ray ) or a CT scan. These x-rays are used to analyse the density of the bone and tissue where the implant will be placed. Local anesthesia is usually adequate for this procedure but for patients who are very nervous and anxious other forms of sedation are available.

Remember to eat and drink fluids (non alcoholic) as normal before all procedures, do not miss breakfast and take all of your medication at normal times.

During the procedure:

Stage 1

An opening is made into the gum where the implant is to be placed and a hole is made into the jawbone. Depending on the density of the bone an adequet sized implant is then surgically inserted to the jawbone. The incision is then stitched and closed. İf there is a denture worn before , the denture will be place over the implant until the implant and settled in place. Sometimes the dentist may not close the gum place a the abutment directly on to the implant and place a temporary crown or adjust the denture to sit on to the abutment.

There is a healing time after this – usually between 8 to 12 weeks minimum. This healing time is for the implant to fuse/connect to the bone and act as a root. Upon succesful intregration (osseointegration), the next stage of the procedure in introduced. Osseointegration in when the bone fuses with implant over a period of time in which then the implant has been successful.

Stage 2

Where the gum has been closed, it is re-opened to expose the implant. An abutment is then screwed and tightened to the implant using special dental equiptment so that it does not come loose. After this is done then a temporary crown is placed on to the abutment. The abutments need to be protected from chewing force to ensure successful healing. The temporary crown is used for a few days until the permanent crowns are ready.

After stage 1, when the implant has been placed there is a healing period of 8 to 12 weeks minimum. During this time osseointegration takes place. The implant fuses to the bone and is ready to act a root and a strong foundation fort he cron or denture.

After the crown or denture has been placed sensitivity may be experienced. Pain killers can be taken and any pain or sensitivity should subside after 2-3 days.

  • Cleaning
  • Successful implant surgery depends on keeping the mouth as clean as possible.
  • You should clean your teeth as normal but avoid the area where the implants have been placed on the day of surgery.
  • Use interedental brushes if a bridge is in place.
  • Rinsing your mouth with warm water and a pinch of salt is great way to help the area heal and keep it clean.
  • Rinse your mouth after eating to keep the area clean.
  • Try not to smoke until the area has healed as smoking limits healing of the mouth.
  • You are advised not to smoke until the wound has healed as this limits healing the mouth.

Swelling is nomal after an implant and can be worse in the morning after lying flat throughout the night. Swelling usually subsides after 48 hours. Swelling can be reduced with ice packs. Place them on the cheek for 10 mins at time with a 15 minute break.

- There may be some bruising on the skin but this will clear up in a few days.
- Avoid hot first drinks or hot food for the first day.
- Do not eat until the local anaesthetic has worn off.
- Try not to rub or touch the area with your tongue or fingers.
- Pain should not be excruciating. Although pain is normal for a few days following surgery it can be easily managed with pain killers. If pain starts getting worse rather than getting better you should contact your dentist.
- Try to wear a denture as little as possible (if it covers the implant area). This will help protect the implant or bone graft. If the denture does not seem to fit properly after your surgery then you should have it adjusted. A badly-fitting denture can damage a healing implant.
- Stitches are normally dissolvable but usually remain for around 2-3 weeks.
- Some bleeding following surgery is normal. If it persists, apply pressure with a dampened gauze for up to 30 minutes.
- If you have been given antibiotics to take after your surgery then complete the full course.
- If you experience increasing pain or swelling after 3-4 days you should schedule an appointment with your dentist.
- A patient should avoid strenuous exercise up to 48 hours post surgery.

I would strongly recommend this dentist to anyone

My Name is Anthony Hicks and I am an American Citizen, living in Berlin, Germany for the past 20 years and have chosen to have all my dental work done with Tourmedical in Kusadasi turkey.

I just like yourself had so may questions and fears about going to another Country and dentist to have my dental work done. At the end of the day I am very pleased and extremely happy with the work and treatment that I have received.

Before I even got started with my treatment I felt comfortable after talking by phone to a very friendly lady named Majella, the patient co-ordinator about what I thought I needed done to my teeth. Majella gave me a quote and told me the dentist will give me a thorough examination and then an exact treatment plan will be drawn up.

I arrived at Izmir Airport amd was greeted by Majella and her husband Omer, the driver, and what a safe and well informed driver he is. They both answered all my questions concerning Kusadasi and surrounding areas. I was taken to my hotel and found it to be very comfortable and clean with a wonderful friendly staff plus a beautiful view of the Aegean Sea. If for any reason, I needed help or guidance the staff at the hotel were very willing to help.

I got to meet my dentist and the rest of the staff the next day. My xray was taken and then I sat with her and she asked me what I wanted her to do for me. I told her and she pointed out what could be done which was a lot more than I had thought, but deep down, I knew my dental situation was not good and knew that every thing she said was true. I am so happy with the result as are my family and friends and they are amazed at how natural my new teeth look. I can say that Dr Elcin worked on my mouth as if it was a piece of art work and I can honestly say today it looks like a master piece.

The dental clean was immaculately clean and i put on plastic blue over shoes as I entered the clinic, as you would if you were entering an operating room!! I was very impressed with the hygiene and how bright the clinic was and the state of the art equipment.

I took my time and researching before choosing Tourmedical an excellent website that is very informative . I have as today 16 implants and 28 zirconium crowns and no residual pain, very amazing. I would strongly recommend this dentist to anyone. I am very very happy not only with the affordable cost but the care, concern and attention I received throughout my treatment by all of the dental team and next year my wife will also have her teeth done.

Thanks to all the team.

I can highly recommend Tourmedical Cosmetic Dental to everybody

My teeth were in very bad condition, and as i work with the public, the time came for me to do something about it. As Ireland was much too expensive for me, I searched the web and found Tourmedical cosmetic holidays, I was very pleased with my 1st point of contact, who was very informative and friendly. I booked my holiday to Kusadasi and was collected from my hotel that evening for my consultation, where my exact planning was done. I needed dental implants, so my appointment day and time was arranged. I was very nervous, but foundthe dentist and her assistants very professional and caring. Their Implant surgeon, was excellent, he was very professional and explained what he was going to do and began the procedure, I didnt feel a thing and was over really quick. I then had to wait 2 months for my implants to fuse with my bone to make them strong enough for my new teeth. I had a temporary denture in the meantime, so could still get on with my life. When i was ready to come back, i contacted them and my appointments were arranged and I was collected for every appointment and taken to the beautiful clinic, which i was really happy to be going back to, all my nerves were gone. Ive now completed my treatment and have a beautiful smile that I thought id never have again. 2nd time round i was treated by the cosmetic dentist and she was brillant, she knew exactly what type of teeth would suit me and I can highly recommend Tourmedical cosmetic dental holidays for any dental problems you may have.

It was the best thing I have ever done

Firstly I have to say I am a very nervous patient and really scared of the dentist, but am proud to say I have come out the other side. Ive completed my treatment now and have to say found the whole procedure as comfortable as possible. From my first point of contact with Toumedical – I was made to feel at ease and guided the whole way. The clinic and technology they have is amazing, I have never been in a place so clean and hygienic. The Cosmetic Dentist is so caring and nice. In my job, your appearance is everything, so I had to be sure I was going to have a perfect smile & the best result. I felt confident from the beginning from our consultation conversation, that the dentist knew what she was doing & I was going to have the Hollywood smile I dreamed of and here I am smiling now to show the world.
